Social prescribers are able to link patients with support services that align with their needs and goals, ensuring a well-rounded approach to healthcare. They work collaboratively with other healthcare professionals to provide patients with a range of options that are tailored to their unique requirements.
You can easily access the social prescribing team by contacting reception directly, or by being referred by anyone within your practice. Upon your consent, a social prescriber will conduct an assessment and work with you to devise a wellbeing plan that addresses your individual needs.

Clinical Pharmacists are an integral part of the healthcare team working within your GP practice. Unlike community Pharmacists who work in pharmacy stores, our Pharmacists work closely with other healthcare professionals to ensure that all aspects of your medication are appropriate, safe, and effective.
Our Pharmacists play a vital role in checking medications added from hospitals and conducting regular reviews of our patients’ medication. They proactively contact patients and engage in discussions about their medication to ensure that it aligns with their needs and goals. They are the best person to speak to for a thorough review of your medication and have their own appointment list.
Our Clinical Pharmacists work diligently in the background to ensure that your prescriptions are safe and effective. They may conduct a telephone review of your medicines, where they will call you and discuss your medication with you. They have access to your appropriate clinical records and work collaboratively with the rest of the healthcare team to provide comprehensive care.
Pharmacy Technicians collaborate with the PCN Clinical Pharmacist and practice staff to ensure the safe, accurate, and timely supply of prescribed medication. They also provide support to other clinical staff and reception teams by coordinating discharge summaries and medication reviews, and play a central role in repeat prescribing processes. Additionally, they liaise with patients, nursing homes, carers, hospitals, and community pharmacies, and respond to medication-related queries.

Nurse Associates perform a similar role to traditional nurses in your practice or home, providing support for a wide range of problems, including dressings, injections, minor illnesses, contraception, and chronic diseases. They work collaboratively with other healthcare professionals to provide comprehensive care to patients.

Physician Associates are integral members of the healthcare team who work within clinical teams to provide high-quality individual care to patients. They have the expertise to treat patients with acute and chronic illnesses and work collaboratively with doctors and nurses as required.
Similar to doctors and nurses, Physician Associates provide assessments, diagnosis and treatment for a wide range of conditions. They work to ensure that patients receive the best possible care and support.
Our Physician Associates work independently in each practice and offer appointments in their clinics. They are committed to providing high-quality healthcare services to patients and work collaboratively with other healthcare professionals to ensure the best possible outcomes.
Our advanced PCN Physiotherapists work in close collaboration with the practice and PCN to provide assessment and treatment for a wide range of conditions that can impact your functioning and mobility. They are highly skilled at diagnosing and treating various conditions and are the first point of contact for patients seeking physiotherapy services.
Physiotherapists play an essential role in supporting patients by providing assessments, treatments and referrals, as required. They are experienced in providing tailored treatment plans that address the unique needs and goals of each patient.
Patients may expect to have a telephone or face-to-face assessment with our Physiotherapist after contacting the practice. They will provide guidance and support on advice and may schedule follow-up appointments, as required. If necessary, they may prescribe and refer patients to more formal courses of physiotherapy for further investigation.
The Care Co-ordinator collaborates closely with the practices and the multidisciplinary team (MDT) in the Primary Care Network (PCN). This role plays a crucial part in ensuring that every patient receives the best care and service. The responsibilities of a Care Coordinator involve various aspects.
They collaborate with General Practitioners (GPs) and other primary care experts in the PCN to identify and oversee patients. When necessary, they refer individuals to other healthcare professionals within the PCN.
They establish strong partnerships with community services and other service providers to enhance coordination and quality of care.
Admiral nurses provide specialist dementia support for individuals and families who are experiencing complexities in their diagnosis or during difficult periods of transition and change. They complete a personalised and holistic assessment of your needs and implement practical, educational and emotional support to ensure that the person living with dementia and those that care for them continue to live well.
